Empowered Nursing Scholarship Raffle Terms and Conditions  The promoter of the Empowered Nursing Scholarship Raffle (the “Raffle”) is Health Carousel, LLC, having its registered office at 4000 Smith Road, Suite 410, Cincinnati, OH 45209 (“Health Carousel”).  The Raffle is open to individuals who: (i) Register for at least one (1) event during the Health Carousel Nursing Careers Week and check the appropriate box on the registration page to be entered into this Raffle; (ii) are a registered nurse (RN) and have passed the NCLEX, if applicable; iii) are legal residents of the United States of America; and (iv) have reached the age of majority, at the time of their entering this Raffle, in the state where they reside, (together, “Entrants”). Individuals residing outside of the United States of America may enter the Raffle under the following additional requirements: if selected, the winner must: (a) provide Health Carousel with a Form W-8, providing their name and permanent address for purposes of informational reporting, and (b) understand and acknowledge that the prize may be subject to United States taxation, in which case the winner may need to apply for a US tax ID and file a US tax return. The winner in this Raffle must meet these eligibility requirements to receive the prize. There is no entry fee and no purchase necessary to enter this Raffle. By entering this Raffle, an entrant is indicating his/her agreement to be bound by these terms and conditions (the “Terms”)

‍Recitals  

1. Entrants will receive one (1) entry upon successful registration for the Raffle. Entrants are limited to one (1) entry. Any additional entry attempts will be void.

2. Entries may be submitted beginning on October 1, 2024 at 12:01 AM (EST) and ending on November 15, 2024 at 11:59 PM (EST). After this date, no further entries to the Raffle will be permitted.

3. Health Carousel shall have no responsibility for entries not received, for whatever reason.

4. Health Carousel reserves the right to cancel or amend the Raffle and these Terms without notice in the event of a catastrophe, war, civil or military disturbance, act of God or any actual or anticipated breach of any applicable law or regulation or any other event outside of Health Carousel’s control.

5. Health Carousel is not responsible for inaccurate prize details supplied to any entrant by any third party connected with this Raffle. In the event of a conflict between any information supplied by a third party and the information contained in these Terms, these Terms shall govern.  

6. One (1) prize will be awarded in this Raffle. Odds of winning depend on the total number of entries. The prize will consist of:

Five Thousand US Dollars ($5,000)

The prize is as stated, and no other alternatives will be offered. The prize is not transferable. Prizes are subject to availability, and Health Carousel reserves the right to substitute any prize with another of equivalent value without giving notice.

7. A winner will be chosen at random from all entries received and verified by Health Carousel and/or its agents. Health Carousel will verify all requirements for entry, including, but not limited to, verification of the winner’s status as a registered nurse and successful passing of the NCLEX, if applicable.

8. The winner will be notified by email within five (5) business days of the closing date. If the winner cannot be contacted or do not claim the prize within fourteen (14) calendar days of notification, Health Carousel reserves the right to withdraw the prize from the winner and pick a replacement winner.

9. Health Carousel will notify the winner when and where the prize can be collected and/or collect information pertaining to where the prize should be delivered. Health Carousel shall not be liable for the winner’s failure to collect and/or provide the appropriate information for delivery or for any delivery issues that may arise as a result of the carrier’s negligence or intentional misconduct.  

10. If you are selected as the winner of this Raffle, the prize payment will be reported to you on a 2024 Form 1099 as non-employee compensation, of which you will receive a copy on or before January 31, 2025. You are responsible for reporting this payment to the IRS and paying any taxes owed on the prize winnings.

11. Health Carousel’s decision in respect to all matters to do with the Raffle are final.

12. By entering this Raffle, an Entrant is indicating his/her agreement to be bound by these Terms.

13. The Raffle and these Terms will be governed by Ohio law and any disputes will be subject to the exclusive jurisdiction of the courts of Hamilton County, Ohio.  

14. By entering this Raffle, an Entrant is consenting to Health Carousel’s utilization of the Entrant’s submitted photo or video, name, likeness, appearance, voice or other personal characteristics, in any medium or format, in connection with the marketing, sales, educational, promotional and/or other endeavors of Health Carousel. In addition, Entrant agrees to allow Health Carousel to digitize, modify, alter, edit or adapt any image, video or recording as necessary.

15. This Raffle is in no way sponsored, endorsed or administered by any company outside of Health Carousel. Entrants are providing information to Health Carousel and not to any other party.

The prize is as stated, and no other alternatives will be offered. The prize is not transferable. Prizes are subject to availability, and Health Carousel reserves the right to substitute any prize with another of equivalent value without giving notice.

37. A winner may only win once. Each winner will be chosen at random from all entries received and verified by Health Carousel and/or its agents for each individual webinar session. Health Carousel will verify all requirements for entry, including, but not limited to, attendance of the webinar session and verification of the winner’s status as a registered nurse.

38. Each winner will be notified by email within twenty-four (24) hours of the end of the applicable webinar session. If the winner cannot be contacted or does not claim the prize within five (5) calendar days of notification, Health Carousel reserves the right to withdraw the prize from the winner and pick a replacement winner.

39. Health Carousel will notify each winner when and where the prize can be collected and/or collect information pertaining to where the prize should be delivered.
